What You'll Do

• Evaluate residents and develop individualized plans of care
• Provide skilled physical therapy focused on mobility, strength, balance, and independence
• Use your clinical judgment to adjust treatment plans based on resident progress
• Educate residents, families, and caregivers
• Collaborate with nursing, therapy, and community teams
• Supervise PTAs as appropriate
• Complete timely clinical documentation

What You'll Need

• Graduate of an accredited Physical Therapy program
• Current state Physical Therapist license
• Current CPR certification
• Strong clinical, communication, and documentation skills
• Home health or senior living experience required
