Job Summary:

We are urgently seeking a highly skilled Physical Therapist (PT) for a FT position in Home Health and Outpatient settings in Naples, FL. The ideal candidate should be knowledgeable, experienced, and passionate about delivering quality care to patients with a wide range of physical conditions. Responsibilities:

Provide screening, evaluation, and treatment of patients with physical impairments or disabilities in both Home Health and Outpatient settings.

Develop individualized care plans and treatment goals based on patient conditions, medical history, and responses to previous treatments.

Implement therapeutic exercises, manual therapy, functional training, and other interventions to improve patient outcomes.

Conduct ongoing assessments of patients to monitor progress and make necessary adjustments to care plans.

Consult with other healthcare professionals, including physicians, nurses, social workers, and occupational therapists, to ensure coordinated and comprehensive care.

Maintain thorough and accurate documentation of patient evaluations, progress, and discharge summaries.

Requirements:

Graduate of an accredited Physical Therapy program.

Current state licensure as a Physical Therapist.

At least one year of experience as a Physical Therapist in Home Health and Outpatient settings.

Strong interpersonal and communication skills.

Ability to work independently and collaboratively as part of a team.

Excellent problem-solving, organizational, and time-management skills.

Availability to work flexible hours, including weekends and evenings.

Physical ability to perform essential job functions, including lifting and positioning patients and equipment.
