Job Title:    Physical Therapist Home Health/Outpatient - NAPLES 

Location:    Naples, FL

Job Summary:

We are urgently seeking a highly skilled Physical Therapist (PT) for a FT position in Home Health and Outpatient settings in Naples, FL. The ideal candidate should be knowledgeable, experienced, and passionate about delivering quality care to patients with a wide range of physical conditions. As a Home Health Physical Therapist, you will have the opportunity to create positive changes in the lives of patients by helping to restore function, improve mobility, and manage pain.

Responsibilities:

• Provide screening, evaluation, and treatment of patients with physical impairments or disabilities in both Home Health and Outpatient settings.
• Develop individualized care plans and treatment goals based on patient conditions, medical history, and responses to previous treatments.
• Implement therapeutic exercises, manual therapy, functional training, and other interventions to improve patient outcomes.
• Conduct ongoing assessments of patients to monitor progress and make necessary adjustments to care plans.
• Consult with other healthcare professionals, including physicians, nurses, social workers, and occupational therapists, to ensure coordinated and comprehensive care.
• Maintain thorough and accurate documentation of patient evaluations, progress, and discharge summaries.

Requirements:

• Graduate of an accredited Physical Therapy program.
• Current state licensure as a Physical Therapist.
• At least one year of experience as a Physical Therapist in Home Health and Outpatient settings.
• Strong interpersonal and communication skills.
• Ability to work independently and collaboratively as part of a team.
• Excellent problem-solving, organizational, and time-management skills.
• Availability to work flexible hours, including weekends and evenings.
• Physical ability to perform essential job functions, including lifting and positioning patients and equipment.
