We are seeking a compassionate and skilled PRN Registered Nurse (RN) Home Health Care Manager to join our team in Plano, Texas, and the surrounding areas.
As a PRN RN Home Health Care Manager, you will play a crucial role in delivering high-quality patient care within home settings. You will be responsible for assessing patient needs, developing care plans, coordinating services, and providing skilled nursing care. This position requires effective communication, strong clinical skills, and the ability to work independently while maintaining a collaborative relationship with interdisciplinary teams.
- Conduct comprehensive patient assessments to determine healthcare needs and develop individualized care plans.
- Perform skilled nursing tasks including medication administration, wound care, IV therapy, and other procedures as necessary.
- Coordinate and monitor patient care and services provided by nursing staff and other care providers.
- Educate patients and their families on health management, medication administration, and wellness strategies.
- Document all patient interactions, care plans, and progress notes accurately and in a timely manner.
- Collaborate with physicians, other healthcare professionals, and support staff to ensure comprehensive and coordinated care.
- Participate in quality improvement initiatives and maintain compliance with state and federal regulations and agency policies.
- Provide on-call support as needed to address patient issues and emergencies.
**Qualifications:**
- Current and valid Registered Nurse (RN) license in the state of Texas.
- Minimum of 2 years of nursing experience, with at least 1 year in home health or community health preferred.
- Strong clinical assessment and critical thinking skills.
- Excellent communication and interpersonal skills, with the ability to educate and motivate patients and families.
- Valid driver’s license and reliable transportation with the ability to travel within the assigned regions.
- Basic Life Support (BLS) certification; CPR certification also preferred.
- Experience in Wellsky EMR a plus.
