Position Summary:

1. Must be available to work a fully in person office position.
2. Coordinates and schedules the daily work load of each district utilizing Agency personnel to their fullest capacity.
3. Provides daily supervision of district nurses and Home Health Aides to ensure the quality of services and maintenance of standards.
4. Reviews and investigates the clinical content, diagnoses, medical history, medications, fee status and other procedures from the records of clients.
5. Directs staff toward the appropriate use of community resources
6. Appraises staff performance during their introductory period and annually with the DON.  Provides the teaching and counseling necessary to enhance the delivery of services and the professional development of nursing and health aide staff.
7. Identifies and evaluates client needs and appropriateness of home care by reviewing the initial intake screening form before assignment of direct care staff.
8. Participates in community related functions as necessary.
9. Assists with the orientation of new professional staff to familiarize them with Agency policies and procedures.
10. Sees clients in the home setting as needed.
11. Participates in audits, U.R. interdisciplinary team conferences and represents agency in the community as requested.
12. Reviews each nurse’s schedule for recertification, discharges, and aide supervisory visits.
13. Reviews weekly schedule submitted by nurses for changes or updates.
14. Fills out the clients’ re-hospitalization referral/admission form sheet daily and submits it to the intake department with recertification date.
15. Performs other duties as directed.
16. May fill in for the other supervisory staff when necessary.
17. Understands and adheres to established policies and procedures of agency.
18. Participates in Quality Assessment and Performance Improvement activities as assigned.
19. Attends all patient care conferences as scheduled.
20. Completes and submits OASIS assessments, reassessments, transfers, resumptions of care, discharges, visit notes and significant change in condition in accordance with agency defined time frames.

Qualifications:1. Valid and current RN license in the state of Florida.2. Bachelor's degree in Nursing (BSN) or Associate degree in Nursing (ADN) from an accredited nursing program.3. At least two years experience in Home Health with 1 year in a supervisory role 4. BLS (Basic Life Support) 5. Excellent interpersonal and communication skills to effectively interact with patients, families, and healthcare professionals.6. Strong critical thinking and problem-solving abilities to make timely and appropriate decisions in emergencies or complex situations.7. Compassionate and patient-centered approach to provide comprehensive care to diverse patient populations.8. Proficiency in electronic medical records (EMR) and computer literacy, familiarity with Kinnser a plus.
