Discovery at Home is searching for an experienced home health scheduler to join our fast growing team and help us maintain efficient operations and high quality patient care. 

This is a full time, in office position which is responsible for coordinating/managing schedules between employees and patients for both skilled and private pay clients. This position works closely with the team to ensure patients are scheduled based on their needs and physician ordered frequency. Reviews staff schedules on a daily basis and notifies the manager of scheduling volume concerns.  The role requires a strong understanding of home health operations, excellent communication skills, and the ability to manage multiple priorities in a fast-paced environment. 

Patient Scheduling Responsibilities:

  • Ensures scheduling is performed in accordance with defined policy and procedures.
  • Strives to achieve continuity of caregivers & field staff for each patient.
  • Communicates scheduling updates and changes to the field staff, patients and families
  • Maintains accurate and up to date schedules in agencies EHR system.
  • Monitor missed or late visits and take proactive steps to resolve scheduling gaps.
  • Provides frequent coordination of care with management team
  • Provide support and assistance to the administrative team within the office (answering phone, making copies, sending emails, filing paperwork, running reports).
  • Tracks attendance for all private pay care staff

Pay: $19.00-$21.00/HR based on experience and skillset

Location: Hillsborough County. 

Qualifications

The ideal candidate for this role will be a professional that excels in a fast paced environment with exceptional customer services skills. The ideal candidate is friendly, organized, a quick learner, confident in communication, proficient in computers/typing.

  • A minimum of 1 year of Home Health/Home Care patient scheduling experience is required.
  • HHA or CNA certification preferred but not required
  • Familiar with Medicare guidelines and private pay service models
  • Experience with EHR systems
  • Excellent communication and problem solving abilities
  • Bilingual (English/Spanish) not required but preferred
